Long chain polyunsaturated fatty acids are transferred from mother to fetus in the third trimester through the placenta, and to infants through breast milk after birth [19]. During the last trimester and neonatal period, brain tissue is rapidly synthesized. Cell differentiation and development of active synapses in the brain need specific requirements of DHA and AA. Eighty percent of brain DHA is acquired from the 26th week of gestation until birth. Notably, the synthesis of AA and DHA from linoleic acid (18:2w6) and alpha-linolenic acid (18:3w3) is limited in the fetus and neonate due to the premature enzyme activity. Thus, the required amounts of AA and DHA must come from the mother during pregnancy, or as breast milk after birth. One study has showed that the fat content and the percentage of all polyunsaturated fatty acids in breast milk increase significantly between the sixth week and sixth month of lactation [20]. There is evidence that slowly turning-over maternal body pools of AA are the major source of milk AA [21]. The AA concentration in breast milk is dose-dependently associated with the consumption of AA-rich foods in lactating mothers [22]. Exclusive breastfeeding for the first six months of life optimizes infant growth, development, and health and is beneficial to maternal health. Breast milk contains adequate nutrients, minerals, vitamins, and water for growing, but for reasons not completely understood, the mammary tissue of most mammals produces large amounts of nonprotein nitrogenous compounds, including free (protein-unbound) amino acids (FAAs) (Armstrong and Yates 1963; Svanberg et al. 1977; Lei et al. 2012).


We obtained a colostrum sample from each woman, transitional milk from 48, and mature milk from 45. There was no significant association between age and missing values for any time point. FAA levels changed significantly with increasing lactation (Table 1). Most FAAs (histidine, isoleucine, leucine, methionine, phenylalanine, threonine, valine, cysteine, tyrosine, alanine, aspartic acid, glutamic acid, glutamine, glycine, serine) increased as lactation progressed, four decreased (lysine, arginine, taurine, proline) and two were undetectable at any time point (tryptophan, asparagine). Glutamic acid, glutamine, taurine and alanine were most abundant in milk at each of the three stages of lactation. Both nonessential and essential FAAs increased as lactation progressed. Maternal age did not have a significant linear relationship with any of the FAAs (Table 1).


Second, aspects of the experimental protocol may have resulted in lower FAA levels. In the present study milk samples were collected after an overnight fast. While the levels of free glutamate in the milk of lactating women collected after 8 h overnight fasting (Stegink et al. 1972) are comparable to the ones found in the present study, levels of FAA in milk appear to change throughout the day (Sánchez et al. 2013). Nevertheless, future studies should be conducted that compare levels of FAA in milk under experimentally controlled fasting and fed conditions. Because animal studies suggest that dietary branched-chain amino acids (BCAA) contribute to the synthesis of free and protein-bound glutamate, glutamine, aspartate and asparagine in milk (Li et al. 2009; Lei et al. 2012), the short-term regulation of FAA content and the effects of dietary history in the short- and long- term in humans are important areas for future research.


Fourteen free-living lactating women participated in a cross-over dietary intervention study, consuming a low fat diet (17.6% of energy as fat, 14.4% of energy as protein, 68.0% of energy as carbohydrate) and a high fat diet (40.3% of energy as fat, 14.4% of energy as protein, 45.3% of energy as carbohydrate) each for periods of 4 days, in randomised order. Each mother was her own control. Mature milk samples were collected during each period and analysed for medium and long chain fatty acids.


The purpose of this study was to determine the effect of short term dietary manipulations of low and high fat diets on the milk fat content and fatty acid content of mature human breast milk in a sample of free-living Canadian women, and specifically, to determine the effect of dietary fat content on medium chain fatty acids (MCFA), and long chain fatty acids (LCFA) linoleic, α-linolenic, docosahexaenoic and arachidonic acids in mature breast milk.


This study investigated the effect of physiologic dietary fat intake of free-living Canadian lactating women on the fatty acid composition of medium and long chain fatty acids in mature breast milk. Using a combination of dietary items reflective of the Canadian diet, women were provided with all their food and energy requirements for two four-day periods. The investigators were confident that the women complied well with the diets as there was continuous interchange and interviews, with an understanding of the importance of consuming only what was provided. In addition, others have demonstrated that participant compliance in controlled dietary intervention studies is typically high [26].

Results from a subgroup analysis from these tucatinib combination studies showed 22 percent of patients with HER2-positive (HER2+) metastatic breast cancer with and without brain metastases achieved prolonged PFS defined as twice the median PFS seen in these studies or else defined as survival greater than 17 months. Patients received a median of two prior HER2-based regimens and the median duration of the last prior treatment in this subgroup was 9.03 months. Patients with prolonged PFS on tucatinib combination therapy included those with characteristics that historically predict poor outcome such as hormone receptor status, presence of visceral disease, burden of systemic or brain disease and age.
